I can write MIDI for you. Some sequencers like Cakewalk can translate it into notes.

Some soundcards recognize MIDI sustain controllers in very mysterious way, so here's version without sustain:

I don't know, is it really starts in D# key or it's just transpose function on keyboard, but sounds like D# ;)
